6 out of 60 is 10%.

So 10% of the queens flew back to their hives after been transferred to a 5
frame nuclei that was left a meter or less infront of it.

60 double box hives (on 10 palets of 3x2 hives) were orphanized so as to -
10 days later - been able to have enough frames with queen cells to make
make 2x or 3x splits while conserving the original queens.

10 days later, on 6 of those hives we found the original queens that have
returned. Of course, on their respective 5 frames nuclei there was no queen
and some queen cells.

Human error? Doubt it, but it could always be so take it as an anecdote.
