Is there any chance that colony superseded a failed queen and sent out a
swarm with one of the virgin queens?

It's been my experience that a superceded queen does not trigger a swarm,
rather the new queen replaces the old and life goes on. You may even see
both the mother and daughter in the hive at one time for a while before the
queen dies.
