Dave Nesky(my partner in crime ;-) and I have observed an increase in EFB to very noticable levels after decades of virtual absence in our operation when we started to dust once every spring with tylan some years ago. Our staff didn't know what they were looking at at first as they never had seen it. I recognized it as EFB and told them not to burn it as we could clean it up with terra. Since then we have returned to a dusting of terra.
We always use dustings with antibiotics. I blame grease patties and subsequent long exposures in hives on much of the resistance issues we have. I recommend minimal treatments with terra or tylosin.
