Sorry for the hassle, guys!  I wanted to require participants to answer the first few questions, but if survey monkey does not accept "other" as an answer that doesn't work for us!  I just changed the survey and it no longer requires that you address those questions.  All "others" should be accepted!

I am very sorry for the confusion and hope that anyone who experienced this problem revisits the survey.  I will also let surveymonkey.com know that they have a bug.
