Time and practice at the breast are most likely to help this baby.
Preterm babies develop the negative pressure component of sucking last,
and develop it earlier if they've had more practice at the breast. There
is a wide variation, some 28 weekers can breastfeed, some 38 weekers
can't. Certainly ruling out tongue-tie is a good thing, and CST is often
very organizing for the nervous system, and that certainly won't hurt.
What I think will be most helpful is for mom to see moving baby to
exclusive breastfeeding as a process that happens gradually. She can
offer brief breastfeedings whenever the baby wants, and then top off
with the fingerfeeding (or a bottle with baby-controlled flow and
stimulation of breastfeeding oral behaviors if that works better for the
family.) As baby transfers more milk from the breast, the breastfeedings
become more numerous and the alternate feedings drop away.
