I can't answer all three questions, but I think I have a handle on the
first one.
Jane Busch tells us, in her article in Historical Archaeology (Vol. 21, No.
1, 1987) , entitled "Second Time Around" on page 75, that the law enacting
the requirement that the words "FEDERAL LAW FORBIDS SALE OR REUSE OF THIS
BOTTLE" went into effect on January 1, 1935. She said there was a big raid
in which millions of bottles were confiscated in mid-January, so my guess
is that bottlers had plenty of warning that this law was going to take
place, despite the way she worded her sentence.
I'll be waiting to hear what others have to say about the the 1965 date.
