Lactnetters,
I'm sure this has been around before. I work in a growing hospital that
is coming out with Breastfeeding Guidelines within the next month. In the
guidelines is written that "no baby being breastfed will receive formula
except for medical reasons", and that no baby will be given bottles unless
there is a written informed consent from mother stating that is OK with her.
My director wonders how this can be implemented. Here's my question to
all you experts in hospital guidelines and their implementation....
What sort of consent form have you used and what wording do you use,
when is it signed, etc. Also, is there someone out there that has charted
the feeding of formula to a breastfeeding infant on the med sheet? If so,
for what reasons. If not, where is it charted and how do you write that
there is consent? Are there legal concerns? (I know there are but am
lacking the correct legal thinking/wording for such for my director)
Anything that you can share with me would be helpful as I am expecting
these guidelines to come out within the month. E-mail me privately unless
this is something everyone wants to get in on. Thanks in advance for all
your help.

the 80's, we're so happy not to be
continuing the record 'highs'...)
