gag order & Risks of Infant Formula:  Hopefully everyone here has copies of
the ICLA HAZARDS OF INFANT FORMULA, but I'm wondering how many of us have a
detailed consent form which outlines these very real hazards?  Is anyone
willing to share theirs?  I have seen the few "consent to supplement" forms
that are in the standard books, but I'm looking for one that really lays it
on the line in terms of statistics (similar to the ILCA statement).  TIA!
