Hi,

I would agree that with a tongue-tie, a baby won't latch correctly; So we
do not have a good oral vacuum, breast not well drained, which is one
thing, and from the autocrine moment, it can be an issue, not well emptied
=> won't fulfill a lot. And this is one part of my patients...
As an example, I have never seen a mom whose infant is in NICU being plenty
of milk with just the baby "not well latched" yet...
As an example, I have never seen a mom with a baby badly latched, and her
body and her brain understand that there is a baby to feed and "hop"
breasts must fill in super quick without any job from the baby....

On the other side, yes, we must think of using too strongly some cranial
nerves/muscles, and not correctly the other structures... which entails
questions about the stimulation of the lips (considering the neural pathway
to hypothalamus);

Maybe the stimulation itself is strengthened on the lips's side because
there is a compensation, and maybe the baby has to feed super frequently
because he/she won't get enough milk in one time, but I am not sure that
this kind of stimulation is enough and correctly understood by
hypothalamus. And then, we go back to: even the "bad stimulation" of "bad
sealed lips" is well understood by hypothalamus and then there is good
oxytocin and prolactin, let's get back to the basic: the breast is not well
empied, so the breast won't fill in again a lot...

I have no lecture to share...

Meanwhile, I can share my practice: I have had 2 kinds of patients...

Those moms who had rapidly induced insufficient milk supply because of the
bad latch, (not because of the use of the breast, frequency or hormonal
issues), and babies needed to be supplemented...That is a good part of my
patients....

And... I have had some moms, whose baby was growing correctly, and
physicians checked nothing because the baby was growing well, and the baby
suffered truly restricted oral tissue + bad latch (each time OT or dentist
would clip and write "posterior type 3") , and : those moms, had large milk
supply, and/or robust ejections....

My thought is that: we all know that even in "normal" situations (no
facial/oral issue), we face moms, some of them have truly robust milk
supply (and they have done nothing for it), and some of them have
truly powerful milk ejection, (and sometimes they do not have
hyperlactation associated).... and "we" need to find strategies for a too
powerful milk ejection, and/or "we" need to find strategies because of
hyper lactation or very robust lactation...

To talk back as far as: what some practitioners say: Maybe there is no link
between "suffering TOT" AND THEN subsequently generating hyper lactation at
all; but maybe there are just circumstances...

Because I have never seen any situation without TOTs, where the body/brain
understands alone that the baby is in need and that the milk should be
synthesized in super good quantity, just as a miracle. (I have also known a
lot moms suffering insufficient milk supply, without TOTs, they all appeal
for a miracle-pill and there is no miracle-pill: good latch and good uses
of the breasts).

I have seen a lot of dyads with tongue-tie , a very lot since 2019 , and I
would be happy to have evidence that TOTs entail super good lactation
too...
