Marcia (and others on the list),
I have posted a PDF copy of one of our recent checklists on our website. You
can download it at
http://www.destinationets.com/resources/printedmaterials/simplesixchecklist.
pdf
We allow the teachers to determine the three levels of marks made: Exposed,
Demonstration, Mastery. That way they can keep their assessment consistant
with what they expect at school (although they are amazed at the
accomplishments of the students during these programs). I have had mostly
great feedback from the form with a few comments that the teachers just
forget to make the marks because they are too engaged in the programs
themselves. A couple of teachers thought that it was a little difficult to
determine which standards were being addressed and therefore didn't finish
the checklist. Hopefully as the become more familiar with the process they
will be able to use the tool.
If you have any other questions send me an email and I will share what I
can.
