We use spreadsheets for most of our employees - filled out by the employee
and turned in as a timesheet. Each one is customized by the projects the
employees are involved in (easy to do with ~20 full-timers).  We do not use
any kind of time verification.

I work on more different projects than most of our employees and I use and
really like LiveTimer (www.livetimer.com)  I pay the $5/month out of my own
pocket because it is so much easier to use that the spreadsheet was, even
when I don't have it running live.  It has saved me about 4 hours per month
of time that used to be spent working on my logs.
