I recently created a job card form for internal use here at Enginuity.
It was fairly simple, no flashy graphics or anything but allow us to
create job cards / work orders for our maintainence team.  The project
was done with HTML, PHP and MySQL.  I can send you screen shots or even
the code off list if you're interested.  Unfortunately, our IT team is
Mircosoft through and through.  The idea of using PHP and MySQL didn't
go over well and the completed and tested project has been shelved
without ever being used.  If you've got any questions about it, let me
know.
