Judy I would recommend bf more frequently like 1-2 hrs. Also bf every 3-4 hrs during nite. More freq. would be even better but most moms do not like to wake sleeping babes. I would suggest a full size supplementer to let baby have what he/she wants/needs - do not hold back on the supplement at this point. Baby will probably nurse better and be more robust when intake is unrestricted. I would continue the br. compression and if she can manage, post feed pumpings at least few times a day to have ebm for use in supplementer. She could try prescription galactagogue like metoclopramide or domperidone whatever is available. Is she on hormonal birth control pills? Is baby on solids.? There are 2 ways of looking at the solids issue: 1. starting too early and giving too much will cut down on br. milk consumption and mat. supply 2. Adding some hi cal solids now if baby developmentally ready could help wt gain. Laurie Wheeler, RN, MN, IBCLC Violet Louisiana, s.e.
