I just got a call from a mom who felt she was not producing milk. Baby was born 3 weeks ago weighing 8lbs. 10oz---now at 9lbs. 8 oz. Mom has been supplementing with abm since baby was 1 week. It seems baby received colostrum---meconium was passed and bm's were typical of breastfed babies. Mom says her milk has always been clear. Never felt fullness, engorgement (even after 6 hours of not nursing) nor any letdowns. Mom nurses baby for 15 minutes on each side than supplements. I suggested getting rid of pacifier for now. Mom says at birth they had to go in and extract placenta. One nurse concerned that fragments may be left---another said no fragments. This mom also states she had no significant breast changes during pregnancy. I urged her to see doctor about possibility of retained placenta and to do a breast assessment. What else? She will be going to the military hospital, and I am not sure if anyone there has experience with any of this. What other suggestions can I give her? Are there any specific tests she should ask for? She is getting out of the military. She will be moving to an area outside of Huntington, West Virginia bordering Ohio the first week in November. Is there an LC there who can help her? Thanks.
