Here is an update on the two cases I presented to you: Case #1: Mom with infertility, pregnancy kept with progesterone and no lactation: TSH (thyroid) normal, Prolacting very high. No retained placenta. Motilium raised production from 10 to 15 ml per pumping. Mother about to quit. My theory: the "progesterone-drop" was not steep enough to sensitize the mammary cells to the effect of prolactin. Does it make sense ? Case #2: 6-months old with stalled weight gain since 3rd month: Mom is now pumping, and is producing quite a bit of milk. Baby is catching up with lightning speed... but is still wriggling at the breast, no matter what the mother does, including spending quiet time with baby, nursing in the dark, while baby half asleep, etc (in fact, the only trick that works a bit is walking while the baby is on the breast... she thinks that the baby feels that he will fall if he moves too much !). Mom is happy, but would certainly prefer quieter feedings. And, of course, she would like to get rid of the SNS, but baby is still very unsettled, as if he will not take the milk directly from the breast. Palate normal, suck normal when he finally sucks on the finger (he looks at you with smile in his eyes, as if saying "what do you want, I am too old for this, can't you see"), but will stick his tongue to the observer more readily on the right than on the left.
