Tell the mother to use the patches and continue breastfeeding.  For
goodness sake, doesn't that doctor understand logic?  Where does he think
the nicotine goes from the cigarettes?  Tell her to ask for the patches and
if the doctor says "You must stop breastfeeding if you use the patches."
Tell her to answer.  "I understand you doctor."

If he pushes, she should just reply, "Doctor, I'll try to stop
breastfeeding, but I don't know if I will be able to quit just right now.
 I do know that I want to stop smoking."

Another sacrifice on the altar of ignorance.
