I just got back from a home visit. Five day old, full term baby girl. Baby was 7lb 4oz at birth and was 6lb 10oz on day four. Nursed well in hospital until second day when baby was "jaundiced" (MD called 14 jaundice) and they kept the baby another day. Fed her glucose water in bottle (argh!) and kept her in the nursery with a pacifier. Baby would then not go to breast so hospital sent mom home finger-feeding EBM. At least mom has a great milk supply and is pumping every two hours and getting about 20 oz. Mom has been finger feeding since discharge. Hospital gave her no instructions about getting baby off finger feeding and on to breast. Baby will not take the breast and is sleepy. She roots very slightly and barely opens her mouth. Mom has large breasts and flat nipples. Tried a "nipple sandwich" and showed her how to put the SNS on the breast. ("Oh! What a clever idea! We never thought of that!" said the family. Why would the nurse give them an SNS to finger feed and never mention putting it on the breast?!?) I was there for an hour and a half and the baby never got the breast in her mouth at all, would cry with frustration and then promptly fall asleep again when comforted. I suggested waking techniques, lots of skin-to-skin, working on opening her mouth, keep trying with SNS at breast. Would this be a situation when you pro-nipple shield ladies would use one? I've never used one before, but after reading all the discussion maybe this would be the time to try one. Any other suggestions? I'll be going back again tomorrow afternoon. Thanks! Cynthia D. Payne
