We were in similar situation, however, I was in my first trimester of my
pregnancy with my daughter.  My son, Lorin, was not quite 3.5yo and only
nursing at night when he came into our bed (yet another variation of the
family bed).  My nipples were quite sensitive and it was uncomfortable to
nurse.  What worked for us was to breastfeed till I counted to 20 or said
the alphabet, then it went down to a count of 10, and finally how about
we'll just cuddle.

It is possible that the three year old you are referring to needs the
physical contact with mom.  Lorin, who is now 5.75yo, still likes to crawl
into our bed at night for cuddle.  Everyone gets enough sleep, though
sometimes I feel like singing "There were four in the bed, and the little
one said, roll over, roll over..."
