Dear Jane, Have you hand expressed the mother? It is amazing how many mothers in my care in the level 2 nursery at which I work just put on the pump, turn it on and pump away with not much result for their efforts. I fully explain physiology and anatomy of lactation and demonstrate by hand exactly where the milk sinuses are situated. I also suggest they begin and end each pumping session in this manner. In most cases the mothers are thrilled to see the milk squirt. It is not a difficult thing to do either with not touching mother. Sometimes that has been an enormous psychological boost for her.( Hand expressing , I mean, not the "no touch") Ruth Leon, RN.RM. IBCLC, NMAA BFC. Sale, Victoria, Australia
