I have have one client with Sheehan syndrome and she could never fully breastfeed. I naturally wondered about the BF management. It was not perfect but then again, it really seemed to be so much more difficult for her to BF that I did not "encourage" her to continue when she expressed her desires to gradually wean. As for the clear blisters on the nipples, I would say it is likely to be herpes unless otherwise proven not to be. So be careful! Could this be why her baby is having breathing problems? If he is already contaminated, couldn't she continue breastfeeding if his condition permits and if her pain is not to intense? R. Lawrence has a goog section on this in her book.
