Several of you have posted about post partum follow up of breastfeeding moms. I send out 2 surveys to the patients that deliver in out hospital that are breastfeeding. The first one is sent out at about the one week mark and the second is sent when the baby is approx one year old. I will share with you a the questions that are on the surveys. BREASTFEEDING SURVEY (1 week) In order to help us provide the best service to our patient, we would appreciate your raking a few minutes to answer our survey reguarding breastfeeding teaching. 1. Please rate the breastfeeding teaching you received while in the hospital. excellent good average fair poor 2. Did the staff give you consistent information on breastfeeding? 3. How can we improve our breastfeeding teaching? 4. Did a LC visit you while you were in the hospital? 5. If yes, do you feel this made a difference in your breastfeeding success? 6. Did you receive any breastfeeding teaching from other sources outside the hospital? 7. If yes, where? 8. Did you receive a phone call from the LC after you went home? 9. Were you interested in attending a BF class before or after you had the baby. 10. If yes, would you prefer: before after both 11. Would you have been interested (for a fee) in having a LC visit you and your baby at home after your discharge from the hospital? 12. Are you aware that the hospital rents Medela electric breast pumps and sells mini pumps, breast shells, nipple cream, BF books and other BF supplies? A space is then left for general comments. Name is optional. This is printed in brochure form and mailed to the mom several days after she goes home. I experimented with putting them in the BF packet of info that I give them but I hardly received any of them back, By mailing them , I think they stand out more and many more of them are returned. Why did you stop BF: (check all that apply) breast/nipple pain, infant fussy/not latching on, concern over baby not getting enough milk, bottle seemed more convenient, lack of support from family, lack of support from doctor/nurses, baby weaned self, decided I had nursed long enough, other____________ 6. Did you want to nurse your baby longer than you did? 7. Would you be interested in taking a prenatal BF class if you had another baby? 8. o you feel that you had adequate information, instruction and support from the hospital reguarding BF?
