Note to Robin, et al on the recent discussion of pacifier use. The parents that come through our hospital and lactation center seem determined to use some sort of artificial nipple whether it is on the bottle or the pacifier. They are not interested in "if" to give it, just "when." I have come up with a 3 point check list for parents to use. They can consider that their baby has probably learned to nurse well after all 3 of these things are true. Pacifier or bottle or whatever can be given then. 1. It is easy to get baby positioned and attached to the breast. 2. There is no nipple or breast pain. 3. Baby is gaining well. I prefer this to a time such as a certain number of weeks. One of the IBCLCs who works with me and who is also a lactnetter, Ann Calandro, came up with a great card addressing pacifier use. On the one side there is a note to mother suggesting rooming in with baby 24 hours and delaying pacifier use until after the above 3 conditions are met. On the other side mother has a check list to indicate her desires regarding baby in the nursery and such. She signs this side and puts it in the crib for staff.
