Sally, my two thoughts on this query are: 1) I've seen babies with tongue-tie, who had been thriving, very suddenly have their growth falter, in one case just during the short time the parents were waiting for an appointment to have the TT clipped. Poor milk transfer. The mother was set up with a home-made supplementer by the child health nurse who saw her during this week or so. This showed me how suddenly this can happen. 2) The pump, a manual one. It is not generally realised that pumps can be faulty, and not generate enough negative pressure (suction). I've seen this with brand new pumps from well-known brands. In this region, I've heard of a spike in faulty manual pumps last year, suggesting poor quality control. However, I've also seen a new middle-range electric pump, reliable brand, that was faulty. I also see second-hand pumps or those used by the mother with a previous baby, where the seals may be ineffective after going through a long hot summer or two - and in Canberra, Sally, you had a very hot summer this year. I took an electric pump and she started expressing more often. The second case was a mother who I had been supporting from day 2, and had seen her establish bf slowly but well, also called me this morning to say breasts no longer dripping (now day 10) and baby very unhappy and screaming at the breast, and not sustaining effective feeds since yesterday.
