Just saw a baby of a similar age and with very similar issues.  The most progress we had has been in positions in which gravity works in favor of keeping baby's lower jaw down - that is the laid back position and baby "sitting upright" and latched straight on with Mom supporting her breast and baby's lower jaw at the same time (Dancer positioning).  This was progress for this nursing pair but they have a long way to go.  I look forward to hearing your ideas -
