When I was actively working as a LC I had a number of clients whose milk had scant to no fat even after being refrigerated. My recommendations were two-fold. I did have the mothers increase the fat in their diets. They were not intentionally restricting fat in their diets but when they kept a food intake diary for a day or two, there was room for improvement. I recommended adding olive oil similar to what Terri describes. I also recommended Neruomins supplement. It is DHA from algae in a base of sunflower oil. There was marked improvement in fat content in all of them in a couple of days. Today I would probably recommend a high quality fish oil which would cover DHA along with EPA. I hold that the fat content in a mother's diet does, in fact, affect the fat content in her milk. The effect most likely varies from person to person as does all nutrient assimilation, but diet matters. She pumped some milk and refrigerated it. When she took it out the next day there was no separation. The last baby she did some block feeding which helped some. She is not on a restricted fat diet. I thought I read somewhere that the content of fat in the mom's diet has no bearing on her fat in the milk. I also suggested marshmallow root. I don't have any evidence to back it up other than mom's saying there milk was creamier and the baby's had settled tummies. I suggested she include olive oil on her salads, bread dipping sauce with olive oil. She has a good milk supply. She just doesn't get much sleep or anything done during the day. Baby falls asleep after 10 min and wants to eat again in 1/2 hour. Baby is one month old next week. His weight is good.
