I think what I would try would be to stop all topical products. I would recommend wound cleansing with a mild soap and water, make sure hands are very clean first and fingernails. Then I would have her apply a very thin application of an antibacterial ointment like bactroban with a clean cotton tipped applicator. I would ask her to do this several times per day. Frequent changes for clean cotton breast pads if she uses them, or making sure she changes bra daily. I think I would also try to get her hcp to prescribe a course of antibiotics which would cover staph and possibly MRSA. I would continue to help with the latching, make sure no pulling or twisting during the feed, in other words support baby and breast throughout. And agree with tips to manage oversupply. If mom can also improve her general nutrition and perhaps a vit C to help wound healing.
