Hi Sonya,
Great history on this mom/baby. So sorry she is in so much pain. Here's
what I would recommend:
Treat for bacterial infection; it is usually staph. However, one might
also culture the milk with a clean catch 
specimen, and culture the reddened skin.
Re-visit the tongue-tie issue. Possibly clipping needs to be revised.
Try "laid back" breastfeeding, where the chin and tongue are more
forward.
Consider CST or chiropractic adjustments which can make things align
functionally and have any nerve impingement
addressed.
Alternatively, the mom could have an allergy to some ingredient she has
been using. I think the tongue-tie started the pain
but an allergy could have definitely made things much worse. In this
case, just the hydrocortisone should help.
