There could be a number of reasons - difficult delivery, torticollis, suck problems, low tone, abnormal tongue movements etc.

Just the other week I was seeing at the hospital a baby who couldn't get deep on and caused mom a great deal of pain. He just couldn't make the proper tongue movements - his tongue didn't go farward, it just curled up to the roof of the mouth and stayed there most of the time, pushed the breast out of his mouth, he kept clamping with gums on the nipple. No tongue tie, but something was wrong with the tongue movements. We refered to a speech-language pathologyst.

If you could describe more what problems the baby you are helping faces, what your observations of the baby are and how the L&D went for this dyad, I am sure you would get more information and hints about what could be going on and how to help this mom and baby.
