Barbara Wilson-Clay likes to use the smallest shield that fits mom's 
nipple to allow the baby to take as much breast as possible beyond the 
teat. I like to use the biggest shield that allows baby to transfer 
milk, so that we can draw as much breast into the teat as possible, 
because many moms allow the baby to suck on the teat, despite!

I've ordered the Mamivac shields from KaWeCo GMBH in Germany (they have 
many sizes, including 28mm), and really like them. I also carry the 
Ameda shields and still have some Medela shields left from when they 
were Code compliant (haven't purchased from them since they became 
violators).
