We are an outpatient clinic. Our initial visits are 60-90 minutes. We try and allot 90, but sometimes it is too busy. If there is less time, it helps to write during the visit. It is also helpful to put baby to the breast after basic/minimal history, and then ask the rest of the history questions while baby is at breast. It is very easy for an initial visit to stretch into 2 hours if parents are anxious and have a lot of questions unrelated to feeding. It is interesting...I had a mom drive an hour from another city the other day to talk to me for an hour about sleep issues and starting solids. I no longer do primary care pediatrics, and I did not ask her, but I assume she chose to drive that far to see me rather than her baby's pediatrician because I am able/willing to spend the time she wants to spend discussing parenting issues. Our healthcare system is so incredibly broken...
