Lactnetter Gonneke van Veldhuizen had an article on this published in the International Breastfeeding Journal, edited by Lisa Amir. She has found that complete drainage of both breasts with a pump followed by breastfeeding several hours on one side only, before switching sides and giving the first breast a break from stimulation, is quite effective in reducing post partum engorgement. Her article contains four case reports and a description of the method she uses. It's freely available on line at http://www.internationalbreastfeedingjournal.com/content/2/1/11 I've recommended this to a number of women plagued by engorgement and oversupply that really made them miserable in the first couple of weeks, and all of them found it more effective than continuing to express just enough to relieve the acute pain. One of them expressed about 500 ml of milk and then immediately afterwards breastfed her 5 kg baby to satiety. After 24 hours of block feeding her engorgement, which had lasted for 9 days with no relief, was gone. Anecdotal, I know - and I would not recommend it except in cases of real, painful engorgement, not as a routine measure for all mothers.
