I do have several kids in my practice who have velopharyngeal insufficiency (poor tone or structure to the soft palate, so it cannot seal off with the pharyngeal walls). They are inefficient feeders, and sometimes even hate feeding. Does this little guy have nasal regurgitation? If milk comes out of his nose, even just occasionally when he vomits, that's a sign that the velum is not doing it's job. Peek under his tongue too. I often see Epstein's pearls in tongue tied babies, especially those with those sneaky posterior ones - tight but far back enough that no one catches it. Usually a baby with a posterior tongue tie will not be able to lift the tongue during crying. I imagine if you could see into the mouth well enough to visualize the uvula while this baby cried, he was not lifting his tongue either.
