She's on Effexor 75 mg po QD, she wanted to breastfeed and her OB had said it would be okay so she didn't switch or wean off the med but then when the baby was born she was told that she couldn't breastfeed, so the baby was not put on the breast at all. Mom feels that the baby has been "looking for the nipple" and she is leaking milk. So, Hale puts Effexor as an L3, we decided to go ahead and try breastfeeding. The baby latched on GREAT! and appeared satisfied after! She is starting out with out jaundice and minimal wt loss, but very infrequent stools b/c of the formula. Mom appears a bit engorged. I told mom to do some warm compresses and to pump to relieve engorgement and help keep up/ bring up her supply. I also demonstrated for her the lactaid (Newman's) We are going to try breastfeeding only today/ tonite and f/u tommorrow to see how it went. I don't trust my scale enough to do pre and post wts since it has been unreliable in the past (a Seca supposed to be accurate to 10 g but repeatability os horrible) My questions are: What can we expect from not having latched at all for the first 5 days? And should the mom try to switch antidepressants to something of a lower classification ie Paxil? By the way- the mom is extremely excited and happy! She said I made her day- she made mine! Amy M.
