I am a PNP/IBCLC and have what is probably the world's best job. I work for Kaiser (HMO) so billing is not an issue. I was hired in the pediatric department specifically to see all our newborns 2-3 days post hospital d/c. I have 20 min. appt. slots and find I am able to do the exam, weight, counseling and BF help for most couplets in that amount of time. I f there is a more extensive problem I do tend to take the time I need and then run behind. If I need more time I can schedule whatever I need at a later date. Because the babies are seen so early some of the big problems don't have time to develop. I see all of these babies at their 2 wk. check also and then they are transferred to their pediatrician. I do BF consults, rent BP's, warm-line BF/NB info, teach prenatal BF classes, and also have a regular panel of older patients that I bonded with during the early visits! I usually see 70+ newborns a month and work about 30+ hours a week. I have an extremely supportive staff of pediatricians. As Pat Gima pointed out NP's can write prescriptions so I can treat thrush, mastitis, etc. Probably the best of all worlds!
