It is easy to do, the mother's hand is in a "U" position with space between thumb and index finger under baby's chin and the thumb on one side of the "jaw hinge" and index finger on the other side. To provide jaw stablity. Kind of like the "U" shape for breast support. Old nursery nurses use this hand position when bottlefeeding a baby that had a poor seal so it can work for breastfeeding. The comment about lack of fat pads in the cheeks is the key here. Early babies or SGA babies don't have the body fat needed on their bodies...anywhere. Maybe a smaller nipple shield would help. Childbirth Graphics has a brochure, written by Sarah Danner that has a picture of this technique. I think it's the one on "Bfing a Neurologically Challenged Baby" The technique was named for Sarah and Dr. Ed Cerutti.
