Diane, Well this may end up as more of an LC job for you, or it may get more and more frustrating. It is hard to see more than 1 "tough case" in an hour. What I would do is LEAVE AFTER ONE HOUR. Then it will get back to you that someone else needed help but you had to leave. Inform the nurses that you are approved for ONLY 1 HOUR and have them document that other patients needed your assistance and expertise. Hopefully they will see that you are needed more than 1 hr, and your other job duties will be cut as you take on more LC work, if that's what you want. Have a few mtgs and document what cases have the highest priority to be seen by you. Off the top of my head, here would be some clients that would need more expertise than the general staff can give: anatomical problems - mother or baby (nipples, tongues) or breast surgeries preterm births or near term babies twins non-latching babies babies who have lost greater than 7% weight One way to provide staff education is to have them come in with you whenever possible to hear the information you give and to see the techniques you use.
