I'm sure you will get answers from lactnetters more experienced with adoptive nursing, but here's my 2 cents: Each situation is different. Your client who is successfully bf w/ the lactaid will make milk, though when that will occur and in what quantities cannot be accurately predicted. *Possibly* she will make more milk sooner w/ the galactagogues and/or extra pumpings, but I would say this was by no means assured. It seems that mothers who have had pregnancies and lactations prior to adopting do have an advantage, but then again, this mother is already SUCCESSFULLY BREASTFEEDING, so I would be inclined to just support that. Of course if the mother has a strong goal of xxx milk production or of decreasing the percentage of artificial milk taken by baby, then using galactagogues and/or pumping might be the thing. I think Darrilyn Starr has had some super-excellent posts on this very subject, so I would be sure to check the archives for these posts. One thing *I think* she said that really stuck with me was something like this "one day mom will make all the milk the baby needs, if she breastfeeds long enough." To me that meant even if a mother makes one ounce of milk in 24 hrs, at some point this will be what the baby/child needs (a 3yo?).
