It sounds as if she was not properly treated for yeast and that she DOES have symptoms of yeast (persistent, burning pain in nipples). First, I would get some really good references for her, such as Jack Newman's and Pat Gima's websites, and share these with her, so she can match her symptoms. Then try to stress to her and dr if necessary the importance of treating both mom and baby at the same time. As you will see on the websites, Nystatin doesn't always lick the yeast. Unless the doc's find something else conclusive, I'm betting it's thrush and she and baby need to be treated (and educated :>) till it's completely gone.
