Even with a mom in ICU, her breasts can be pumped without her having to do anything. Staff or family can hold the flanges in place. Of course, it's easier if mom can sit up straighter, but even if a little is spilled because she is flat on her back, some colostrum is better than none and when she is able, she won't have to "catch up" as much as if there had been no breast stimulation. I have both seen and heard, though, that in some cases when mom is very ill, her physical condition seems to delay milk production.
