I think the reason hospitals continue as they do is just that....moms want to
just get their baby home.  They don't all have the support or information
they need, very very few have the voice they need even if they have the
information.  So, instead, they just "do as I am told" and "obey" so they can
get their baby home to safety.  Sad, but true.  I agree that in the US the
only way we will see a drastic turn around in the care of mothers and infants
is a major lawsuit.  I hope one of you plan on having a baby so you can do
it, because I quit at 6, and I don't even want to go back to a hospital birth
for the adventure.   (I have often threatened to change pediatricians and go
to this scary man who really should not even talk to mothers who breastfeed,
just so I can make his life miserable while I educate him, but truly, don't
have the energy.)
