I am working with a new Mom of almost 3 week old baby, born at 37 weeks gest. Mom had spontaneous vag del, no meds, baby has nursed well, to put in a nut shell "the perfect breastfeeding baby" at discharge. Well, baby had not been gaining, remaining at 12% below bw at 14 days old. Mom is now supplementing at the breast with an SNS, taking fenugreek and post pumping. When taking a history from this Mom, she mentioned she did not notice any change in her breasts during pregnancy and very little when she felt her milk came in on the 4th day. Knowing this may be a red flag for mammary gladular hypoplasia, is there any other way of knowing that this is the problem? What about the recent post where it can take up to 4 weeks for a mom to achieve a full milk supply (with insuf. glan. tissue)? Is this the max or average? Do we know? Are there tests one can have done to know if one has insuf. glan. tis.? I guess I'm asking, at what point does one say, ok, I've done all I can do and X amount of time has gone by and this is all I'm going to produce for this baby? Waiting to hear from those far more experienced than I.
