Without knowing the specific cases, I can almost confidently answer:  they
are if their subscribers are paying premiums that include it, and they are
not if not.    They are not charitable organizations, and their customers are
price sensitive.   Big employers negotiate with their insurers about exactly
how much their premiums will be and what they will get for them.   If lots of
their employees want pumps --- or eyeglasses, or mental health care -- so
much that they are willing to pay an increased premium throughout the
organization to cover it, then it will be in.

It's true of course that  the inclusion of pumps helps normalize bf (or at
least pumping...).   It's even more true, however, that until its already
normalized there won't be any reason for employers or insurers to suggest
putting it into plans.

But the underlying question is whether everybody -- society, let's say, or at
least the society of one's fellow employees -- should cover the costs of your
pump, or whether the family using it should cover it.    Seems to me that
reasonable people could disagree on this.
