Regarding an adult using her/his finger as a pacifier:  I have found
that letting baby suck on a knuckle (I especially liked to use the
little finger) would help to tide baby over on those occasions where
I needed to wait a couple minutes for one reason or another.  The
knuckle doesn't go as far into baby's mouth as a finger.  This
avoids the problem of giving instant stimulation in the back of teh
mouth.  This, I believe, is a key reason babies can become lazy at
brest after bottle/pacifier use.  They expect to feel mom's nipple
in the back of their mouth right away instead of being willing to
suck the few times it takes to draw the nipple in that far.
