Hello Wise Women, I have two difficult maststis cases I'm working with right now. Wonder if I'm missing something? Case #1: 10 months post birth and has had seven bouts of treated mastitis. It goes away and then returns a few weeks later. Doc has been prescribing Keflex. No risk factors other than hx of Beta Strep that was treated at delivery. I am wondering if babe is colonized with beta strep and giving it back to mom all the time. The mastitis moves around from breast to breast and location to location. No anemia. No diabetes.Breasts are soft with localized redness and tenderness in one area at this time. Starting to chill as I was seeing her. Babe nurses in varying positions. I sent her to the pediatrician to get the baby swabbed. Anything else I should have done? Case #2: 3 1/2 weeks post delivery. Has now had mastitis in varying severities for 2 weeks. Is starting a third week of Keflex. Bilateral. Redness and warmth over approx 1/2 of right breast and 1/4 of left. Still having low grade fevers (initially was 104.0 but now about 100.2). I sent her back to the OB to ask for a different antibiotic. Keflex is clearly not getting it. Breasts are very full and heavy so she is also pumping to keep breasts well drained and baby is feeding just fine and thriving. No hx of Beta Strep at delivery. What am I missing?
