My biggest complaint is that the more a mom "needs" them (i.e. the flatter her nipples) the harder they are to keep "on target".  They tend to slip around in the bra so that they are applying pressure anywhere except where they are supposed to be helping.  The only possible use I see is to keep clothing off healing nipples.
