I am so grateful to those who replied. Baby was born in late March, full
term girl. Nursed "great" (Mom's quote) from the get-go. A little bump in
the road when bili was hi and Mom was pumping and supplementing with her
milk (Dr.s orders) for days 3,4,5 or so of baby's life. By day 7 bili
level was lowered, baby was nursing about every 2 hours, had gained 4 oz.
since d/c and mother was recovering nicely. Mother stated that last month
baby's pediatrician was "concerned about milk supply, since wt. gain is
only 60% of what it had been" and suggested fenugreek. It has been a
frustration that mother, although willing to pump more often at work, has
not followed up on suggestion to replace/update her pump despite the
evidence that her pump may be (have been) the problem all along. When
mother and baby are together, baby has always been an avid nurser and up
until last month had been "gaining fine". Mom has a Pump in Style (hope
its okay to name names) which ordinarily would be a fine pump choice for
her, but she got it on e-Bay. (I know, I know...). The only new thing I
could suggest is that if her dr. is willing to prescribe Reglan, perhaps
he can write a prescription letter to her insurance co. to allow for
reimbursement of a rented hospital-grade pump. Not sure of the financial
situation except that they are just over the qualifiers for WIC. I am
leaning toward stress in the house and the inadequate pump as the two
driving factors here. I just wanted to make sure I covered all the bases
since I am studying for IBCLC after being a peer counselor for many years.
I listen carefully to the mom and try to suggest/advise what will work for
her, but I am not very experienced in clinical matters and therefore
always afraid that I may miss something "wrong with the baby" in my
inexperience.
