This happens more and more now that pediatricians have heard of tongue-tie,  
recognize some of really obvious ones but remain ignorant of how to  really 
assess tongue function. "It's just a small one, it won't make a  difference" 
they say by looking at the tongue, usually at rest. I said to the  parents, "it 
IS making a difference, your baby is born to breastfeed and is not  
breastfeeding. It is already making a difference." These are tongues which may  have 
"little frenulums" ( yes they look small or moderate) but sometimes the  further 
back they are, the smaller they look, and the tighter they are. Some  really 
obvious ones are as elastic as those skinny red rubber bands, some of the  
"little" ones in the mid or back area, are as inelastic as the white bands on  
broccoli in the market, or even less so. They can feel like tendons and have no  
give. They can also be small-looking, meaning short, but be weblike and 
restrict  vast areas of tongue. Often these babies are poor assistive device feeders 
too:  they lose milk, they gag easily, get wet-sounding breathing as feeding  
progressed. We can point out that this is not a "breastfeeding issue" this is 
a  "feeding issue, affecting breathing, and comfort and energy use. " As 
Jennifer  said, we can tell the parents all this, send them to good people ( or try 
to),  and offer to put them in touch with people who have gone through the 
same thing.  Some still refuse or go to someone who will support their babies 
feeding issues  without ever addressing the frenulum directly. They just can't 
hear it. All we  can do is share with them our information and support them in 
using it.
