If the baby takes a full feeding in a reasonable time with a nipple
shield, I do not have the mom "insurance pump".  If the shield prevents
the baby from being fully effective at breast, I do recommend pumping
and giving the extra milk.  If the baby does not move much milk with the
shield, I do not leave it with the mother.

This informal policy developed as mothers who were advised to insurance
pump had months of plugged ducts, etc from oversupply...
