Which ML tincture?    A mom who contacted me recently had been taking 
fenugreek capsules without much apparent difference.  When I encouraged 
her to start More Milk Plus (has fenugreek in it)....it was another 
several days before she could get the Goat's Rue which was at a 
different store.  When she added the Goat's Rue, her breasts 
dramatically increased in size and she was immediately able to go from 4 
oz of formula (q 3h) in the SNS to 2.5.  Now, she's doing a couple 
ounces per day.   It's on the way out.   Since she seems comfortable 
with the herbs, i'd really encourage MMP *and* Goat's Rue.   Other moms 
who've used both seem to report better results with the MMP than when 
taking fenugreek in capsule form.

More Milk

Take 3-4 drops per ten pounds of nursing mothers body weight in 1-2 
ounces of water or juice every other hour. In 24-48 hours adjust daily

Check her dosing to make sure she's taking enough of the MM or MMP.  
Make sure she's not taking the MM2 - it's the one that's formulated for 
use during pregnancy.  The benefits wouldn't come close to matching MMP.

Anything about baby's mouth that might make it harder for her to suck 
effectively?  Palate issues of any kind?  I'd work on a really deep 
asymetrical latch - really digging that bottom jaw in and 'rolling'  the 
latch on from there.  Of course, those are probably two of the first 
things you did, but they're the ones that come to mind right now.

So mom is getting milk from pumping post feeds?  How much milk is mom 
getting from pumping post feeds?

It could very well be that just by getting this baby back on track, 
supplementing EBM at the breast - she will be able to go back to 
exclusive breastfeeding.    The mom I wrote about a couple of weeks ago 
is almost there....and her supply was exceptionally compromised...with 
no apparant explanation of how it got that way.   I my mom's situation 
though, her supply was so compromised that formula was already being 
used in significant quantity....and she was getting *zero* from 
pumping.  So she quit pumping, quit bottles, and just simply nursed and 
nursed and nursed: ABM supplementation intermittently, meticulous 
attention to a deep asymetrical latch, breast compression (three 
seconds, then release), MMP/Goat's Rue.  His palate is exceptionally 
high and really scooped out - like a little scooped out kiwi.

Best wishes to the mom...hope it turns around soon.  Very very soon.
