One quick suggestion re pumping; have you had her try Medela's glass flange?  
Sometimes it helps as there is little or no friction as it brings in more 
breast tissue.  Just had a mom today who'd gone up to the 36 mm new flanges and 
still complained of pain (large breasted) and tried the glass one I have a 
smiled and said it was a totally pain free experience with no breast tissue 
rubbing.
